💼 About This Role

You'll provide comprehensive, client-centered case management with a harm reduction focus to individuals who use substances. You'll facilitate access to health services and re-engage participants newly diagnosed or out of care through outreach, testing, and support groups. This role offers the chance to make a direct impact in a nationally recognized center for social justice.

🎯 What You'll Do

• Conduct client intakes and assessments to identify strengths and barriers.
• Develop and update individualized care plans with clients.
• Coordinate internal and external services including medical and housing.
• Facilitate harm reduction and health education support groups.

📋 Requirements

• Bachelor's degree in social work, public health or related field.
• 2+ years providing harm reduction focused case management to high-risk populations.
• Extensive knowledge of prevention and treatment of HCV, HIV, and STIs.
• Ability to plan and implement outreach objectives.

✨ Nice to Have

• California Rapid HIV/HCV/STI test counselor certification.
• Current California driver's license.
• Certified Phlebotomist.
