💼 About This Role

You'll provide home health speech therapy services to adult and geriatric patients in Vance and Warren counties. Your evaluations and treatment plans will help clients remain independent at home while collaborating with a multi-disciplinary care team.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform evaluations for speech, language, communication, or swallowing difficulties.
• Develop and revise treatment plans based on evaluation and physician orders.
• Educate patients and families on rehabilitative care activities.

📋 Requirements

• Current Speech Language Pathologist license in North Carolina.
• Certificate of Clinical Competence or equivalent educational/work experience.
• Solid computer skills for documentation on touch pad tablet.

✨ Nice to Have

• Prior home care experience as an SLP.
• Experience with electronic medical records (EMR).
