Description

You will provide home health occupational therapy to adult and geriatric patients in Indianapolis, performing evaluations, developing care plans, and educating patients and families. Be part of a multidisciplinary team dedicated to helping clients remain at home with rehabilitative care.

Requirements

• Current Indiana Occupational Therapist license
• Graduation from an accredited occupational therapy program
• Minimum one year recent work experience as an Occupational Therapist
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Make home visits to clients in designated territories
• Perform evaluations of functional status and develop individualized intervention plans
• Assess home environment for hazards and identify equipment needs
• Educate patients and family members on rehabilitative care and activities
• Document patient care using electronic touch pad tablet
