💼 About This Role

You'll conduct assessments and provide case management for individuals reentering the community from incarceration, helping them access housing, mental health, and substance abuse services. You'll coordinate with a multi-disciplinary team to prevent recidivism and support long-term stability. This role offers the chance to work directly with clients in field settings.

🎯 What You'll Do

• Interview and assess participant needs
• Conduct wellness checks and provide crisis management
• Develop treatment plans with participants
• Coordinate referrals to housing, mental health, and substance abuse services

📋 Requirements

• Bachelor's degree in behavioral health or social science with 1+ year experience, or high school diploma with 3+ years experience
• Experience in outreach, substance abuse counseling, reentry services, and case management
• Valid driver's license, insured vehicle, and ability to use personal vehicle for work
