🎯 What You'll Do

• Maintain caseload using evidence-based engagement strategies
• Conduct telephonic and SMS outreach for timely follow-up
• Collaborate with care team on delegated tasks and case reviews
• Assist members with community referrals and appointment scheduling

📋 Requirements

• 1+ year of general work experience
• Physically located in California
• Reliable transportation for occasional field visits
• Bilingual in English and Spanish
