Description

You will provide after-hours hospice nursing care to patients and families in their homes, conducting assessments, administering medications, and coordinating care with the interdisciplinary team. This rotating schedule offers one week on (Friday 5 PM through Monday 8:30 AM) followed by one full week off.

Requirements

• Bachelor's or Associate's degree in nursing or related field.
• Active nursing license in good standing in Indiana.
• Active CPR certification.
• 2-5 years experience in healthcare management, hospice, or palliative care preferred.
• 1 year experience with Medicare Hospice Certification and CHAP guidelines preferred.

Responsibilities

• Complete initial and comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process and health assessment data.
• Develop and revise care plans in collaboration with patients and families.
• Administer medications and treatments as prescribed by the physician.
• Communicate with physicians regarding patient needs and changes in condition.
