Description

You will provide hospice visits to patients in their homes, following a designated care plan. This includes administering medication, wound care, treatments, procedures, and monitoring patient conditions. You will collaborate with the clinical team and document care using a touch pad tablet.

Requirements

• Current RN license in South Carolina
• Minimum 1 year recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Ability to work independently and manage time effectively
• Ability to travel to cases

Responsibilities

• Follow designated care plan per patient needs
• Make home visits in designated geographic territories
• Administer medication, wound care, treatments, and procedures
• Monitor client conditions and report changes
• Perform client assessments and case management
