💼 About This Role

You'll join an interdisciplinary team providing virtual outpatient palliative care to people with cancer. You'll lead case conferences and educate staff on symptom management and advance care planning. This role offers the chance to shape a growing program at a tech-forward value-based oncology company.

🎯 What You'll Do

• Conduct virtual palliative care outpatient visits for members
• Lead interdisciplinary case conferences for advanced serious illness
• Provide clinical oversight to case management team
• Educate staff on symptom management and advance care planning

📋 Requirements

• 3+ years NP experience with 2+ years in palliative care
• Advanced Certified Hospice and Palliative Nurse (ACHPN) certification
• Compact RN license and unrestricted NP license
• Willingness to obtain additional state licenses
