Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in senior living communities, performing evaluations and therapeutic interventions for speech, language, communication, or swallowing difficulties as part of a multidisciplinary team.

Requirements

• Current license as a Speech Language Pathologist in the state
• Certificate of clinical competence in speech from ASHA or equivalent educational requirements
• Solid computer skills; prior experience with electronic medical records preferred
• Prior home care experience as a Speech Language Pathologist preferred but not required

Responsibilities

• Make home visits to clients in designated geographic territories
• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for speech, language, communication, or swallowing difficulties
• Develop a treatment plan based on evaluation, physician's orders, and recommended goals
• Participate in multidisciplinary care plan and revise therapy care plan as appropriate
• Educate and instruct patients and family members in rehabilitative care and activities
