💼 About This Role

You'll provide hands-on patient care in home health settings across the Sacramento area, developing skills for case management advancement. You'll work under a Case Manager RN to ensure high-quality outcomes for a recognized Top Workplace.

🎯 What You'll Do

• Perform skilled nursing procedures like wound care and IV management
• Implement care plans directed by Case Manager RN
• Document patient care accurately within 24-48 hours
• Educate patients and families on diagnosis and self-care

📋 Requirements

• Current CA RN license
• 1 year of recent nursing experience
• Valid CA driver's license with acceptable record
• Current BLS certification
