💼 About This Role

You'll join the largest home health agency in San Francisco and provide direct patient care in their homes. This entry-level role develops foundational nursing skills for a Case Manager path. You'll work in the Piedmont coverage area with a supportive team that achieved a 2024 Top Workplace award.

🎯 What You'll Do

• Provide hands-on patient care including PICC lines, wound vacs, and tube management.
• Implement plans of care directed by the Case Management Registered Nurse.
• Document accurately within 24-48 hours per plan of care.
• Attend weekly Case Conferences to coordinate patient care.

📋 Requirements

• One year of recent nursing experience
• Current CA Registered Nurse License
• Current Basic Life Support (BLS) Card
• Valid CA Driver's License with acceptable record
