💼 About This Role

You'll deliver chronic care management to high-complexity patients primarily in their homes and communities, building relationships and coordinating care. You'll guide patients through enrollment, facilitate virtual appointments, and ensure adherence to care plans to improve outcomes. 📋 Requirements
