Description

You will provide home health visits to adult and senior clients, performing assessments, administering medication, wound care, and treatments. You will follow care plans, monitor conditions, document using a touch pad tablet, and coordinate care.

Requirements

• Current Registered Nurse license in MA
• Minimum one year recent clinical (medical/surgical) experience
• Graduation from accredited nursing program
• Ability to travel to cases
• Solid computer skills; EMR experience preferred

Responsibilities

• Follow designated care plan per patient needs
• Make home visits to clients in designated territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes
• Perform client assessments and case management
