💼 About This Role

You'll join a Care and Case Management team to provide telephonic holistic care for members with complex Medical and Behavioral Health needs. You'll create cohesive care plans and guide members through their healthcare journey, ensuring excellent outcomes.

🎯 What You'll Do

• Deliver virtual Care Management via telephone and video.
• Create impactful care plans with members and care team.
• Help members navigate complex medical conditions and benefits.
• Coordinate resources addressing clinical and social needs.

📋 Requirements

• BSN (Bachelor of Science in Nursing).
• 5+ years of nursing experience.
• 2+ years in care management.
• Active Compact RN license in good standing.

✨ Nice to Have

• CCM Certification or eligibility within 6 months.
• Spanish speaking ability.
• Experience working remotely.
