Description

In this hybrid role, you will oversee clinical quality and medical staff across a region of clinics in Houston, while also providing direct patient care part-time. You'll lead initiatives in quality improvement, care coordination, and population health management within a value-based care model.

Requirements

• MD or DO from accredited medical school
• Active Texas medical license
• Board certified or board eligible
• 5+ years clinical experience in primary care
• 3+ years in senior leadership role

Responsibilities

• Supervise and mentor medical providers in the region
• Implement quality improvement initiatives and monitor dashboards
• Develop protocols for seamless care transitions between primary care and specialists
• Design population health programs focusing on chronic conditions and preventive care
• Lead communication with stakeholders and support provider leadership development
