Description

You will independently manage complex cardiovascular patients in a collaborative environment, focusing on hypertension, CAD, and anticoagulation therapy. This role is ideal for confident providers who value autonomy and evidence-based care.

Requirements

• Active, unrestricted NP or PA license in Arizona
• 2+ years of cardiology or advanced chronic disease management experience
• Strong diagnostic and clinical decision-making skills
• Confident independent operator
• Excellent communication and referral relationship management skills

Responsibilities

• Independently manage hypertension, CAD, CVA, dyslipidemia, PVD, PAD
• Oversee and adjust anticoagulation therapy including Coumadin
• Perform comprehensive cardiovascular assessments and medication management
• Coordinate with internal providers and external referral partners
• Maintain clear communication with community stakeholders
