💼 About This Role

You'll provide rehabilitative care through home health visits for adult and geriatric patients. You'll perform evaluations, develop individualized intervention plans, and identify equipment needs to promote independent living in a multidisciplinary team. 🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of functional status and ADL/IADL performance.
• Develop and revise individualized occupational therapy intervention plans.
• Educate patients and families on rehabilitative care and activities.

📋 Requirements

• Current state Occupational Therapist license.
• Graduation from an accredited occupational therapy program.
• Minimum one year of recent work experience as an OT.
