💼 About This Role

You'll provide skilled home health physical therapy to help patients regain independence after illness or injury. You'll develop care plans, coordinate with physicians, and collaborate with PTAs to deliver high-quality care in the Hayward area. This role offers autonomy with flexible scheduling and targeted zip-code-based territories to minimize driving.

🎯 What You'll Do

• Perform in-home evaluations and develop care plans
• Treat patients with mobility and movement exercises
• Coordinate with physicians and report patient progress
• Supervise Physical Therapy Assistants per regulations

📋 Requirements

• Current unrestricted California PT License
• Current BLS/CPR Certification
• Valid California Driver's License
• Knowledge of local community resources
