You'll provide patient-centered care to residents in skilled nursing facilities, conducting daily rounding and managing treatment plans. Your work builds trust and continuity with vulnerable populations over time. This role offers flexible scheduling and a supportive team environment.

🎯 What You'll Do

• Conduct daily rounding in skilled nursing facilities
• Prescribe medications and treatment plans
• Address acute and emergent issues
• Communicate with facility nursing staff via phone or secure messaging

📋 Requirements

• Active RN license with board certification as NP (ANCC, AANPCB, PNCB, or AACN) or PA (NCCPA)
• Qualified specialties: Internal Medicine, Family Medicine, Emergency Medicine, General Practice, PM&R, or Geriatric Medicine
• Ability to perform essential functions with or without reasonable accommodation

• Experience in skilled nursing or post-acute care
• Familiarity with EHR and voice dictation tools
