💼 About This Role

You'll provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes. You will be an integral member of a multi-disciplinary health care team, making home visits and developing individualized care plans to promote independent living.

🎯 What You'll Do

• Perform diagnostic tests and measurements for mobility, strength, and balance
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Educate clients and family members on rehabilitative care and independent living
• Document observations and interventions using a touch pad tablet

📋 Requirements

• Current MD Physical Therapist license
• Minimum one year of recent work experience as a Physical Therapist
• Graduation from an APTA-approved program

• Prior experience with electronic medical records (EMR)
• Strong interpersonal and problem-solving skills
• Solid computer skills
