Description

You will provide rehabilitative occupational therapy to adult and geriatric patients in their homes, conducting evaluations, developing care plans, and educating patients and families to promote independence and safety.

Requirements

• Current North Carolina Occupational Therapist license
• Graduation from an accredited occupational therapy program
• Minimum one year of recent work experience as an OT
• Ability to work independently and manage time effectively
• Solid computer skills; EMR experience preferred

Responsibilities

• Perform home visits and evaluate patients' functional status
• Develop individualized intervention plans to achieve favorable outcomes
• Assess home environment for hazards and recommend adaptations
• Educate patients and families on rehabilitative care
• Document patient care using electronic tablets
