Description

As a Community Companion Home Coordinator, you will lead and support Home Care Providers in Connecticut to ensure the health, safety, and well-being of individuals in Shared Living arrangements. You will build trusting relationships, collaborate with stakeholders, and help individuals grow and connect in their communities.

Requirements

• Bachelor’s degree or equivalent professional experience
• Prior experience supporting individuals with disabilities strongly preferred
• Reliable transportation, valid driver’s license, and willingness to travel
• Strong communication skills, empathy, and enthusiasm
• Basic computer skills (experience with Therap is a plus)

Responsibilities

• Provide leadership, oversight, and hands-on support to Home Care Providers
• Ensure health, safety, and overall well-being of individuals in Shared Living arrangements
• Travel throughout Connecticut visiting homes, meeting with providers, and connecting with individuals in their communities
• Build strong, trusting relationships with providers and individuals
• Collaborate with management, state agencies, family members, medical professionals, and other stakeholders
