💼 About This Role

You'll join a multidisciplinary home health care team providing rehabilitative care to clients in their homes. You'll perform diagnostic tests, develop individualized care plans, and educate patients and families. This role offers flexibility with per diem, part-time, or full-time options in Bushkill, PA.

🎯 What You'll Do

• Perform diagnostic tests and measurements (mobility, strength, balance).
• Develop and implement individualized care plans.
• Educate clients and families on rehabilitative care.
• Document observations and interventions using a touch pad tablet.

📋 Requirements

• Current Pennsylvania Physical Therapist license.
• One year of recent work experience as a Physical Therapist.
• Graduation from an APTA-accredited program.
• Ability to work independently.

• Prior home health experience.
