Description

As a home care Physical Therapist, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients in their homes. You will conduct diagnostic tests, develop care plans, and educate clients and families to promote independent living.

Requirements

• Current Arizona Physical Therapist license
• Minimum one year of recent experience as a Physical Therapist
• Graduation from an APTA-approved program
• Strong interpersonal and organizational skills
• Computer proficiency; EMR experience preferred

Responsibilities

• Conduct diagnostic tests and measurements (mobility, strength, balance)
• Implement wound prevention and repair interventions
• Develop and implement individualized care plans with manual therapy and gait training
• Educate clients and families on rehabilitative care and safety
• Document care and outcomes using electronic medical records
