💼 About This Role

You'll provide hands-on therapeutic interventions in a home health setting under the supervision of a PT Case Manager. You'll build core clinical skills while contributing to San Francisco's largest home health agency with a 2024 Top Workplace recognition.

🎯 What You'll Do

• Conduct patient evaluations and assessments in the home environment
• Implement individualized plans of care under PT Case Manager supervision
• Provide accurate, timely documentation within 24-48 hours
• Educate patients and families on functional mobility and safety techniques

📋 Requirements

• Current, unrestricted California Physical Therapist License
• Valid BLS/CPR Certification
• Valid CA Driver’s License with clean driving record
• Graduate of an accredited Physical Therapy program

✨ Nice to Have

• One year of Physical Therapist experience
• Prior home health experience
• Experience treating high acuity patients

🚩 Heads Up

• Entry-level role but requires valid PT license and BLS/CPR
• Position is field-based visiting patients in homes with potential travel
• On-call or weekend coverage rotation required
