💼 About This Role

You'll lead clinical operations for a PACE program, overseeing nursing staff and managing care for older adults aging at home. Your impact includes coordinating comprehensive assessments and care plans to ensure participant safety and quality of life. You'll work in a culturally responsive center serving low-income seniors in San Leandro.

🎯 What You'll Do

• Supervise nursing staff and manage daily clinic operations.
• Conduct nursing assessments and develop care plans.
• Coordinate care transitions across settings.
• Provide direct nursing care and triage symptoms.
• Support quality improvement and compliance activities.

📋 Requirements

• Current RN license in California.
• 2+ years experience with frail or elderly population.
• Ability to document in EHR and use standard office software.
• Current CPR/BLS certification.

✨ Nice to Have

• 3+ years RN experience with medically complex older adults.
• Care management experience in capitated models.
• Bilingual ability (Cantonese, Mandarin, Spanish).
