You'll manage a caseload of individuals with serious mental illness or substance use disorder, using relationship-based strategies to promote engagement and health goals. You'll collaborate with a multidisciplinary care team to coordinate whole-person care and reduce emergency visits. This role primarily works from home with 2-3 community visits per month.

🎯 What You'll Do

• Maintain caseload using evidence-based approaches to achieve health goals
• Conduct telephonic and SMS outreach for timely follow-up
• Help members identify health goals and create care plans
• Collaborate with care team on systematic case reviews

📋 Requirements

• 1+ years of case management experience
• Bilingual in English and Spanish
• Valid driver's license and auto insurance
• Located in California with reliable transportation
