🎯 What You'll Do

• Provide primary care and metabolic health management for adults
• Share a patient panel with MD/DO and other APCs
• Coordinate with the broader care team to ensure patient support
• Document accurately and review remote patient monitoring data

📋 Requirements

• Master's degree in Nursing or Physician Assistant studies with relevant certification
• Unrestricted DEA licensure and prescriptive authority
• At least 2 years of clinical experience in ambulatory care
• Active NY license or willingness to obtain

• Experience in chronic condition management
• Weight management training or certification
• Experience with Athena EMR
