Description

You will provide home-based physical therapy to patients in Gloucester County, developing individualized care plans, performing diagnostic tests, and educating patients and families to promote independent living.

Requirements

• Active and current PT license in the state of New Jersey
• Ability to travel to cases as assigned
• Strong assessment, communication, and active listening skills
• Compassionate and reliable patient care approach

Responsibilities

• Perform home visits and diagnostic tests including mobility, balance, and strength assessments
• Implement wound prevention and repair interventions using evidence-based dressings
• Develop and implement individualized care plans with therapeutic exercises and gait training
• Educate clients and families on rehabilitative care for safety and independent living
• Document observations and interventions using a touch pad tablet
