💼 About This Role

You'll provide skilled home health physical therapy to patients in the Galt area, restoring function and promoting independence in a supportive team environment. You'll create personalized care plans and coordinate with physicians to achieve outstanding outcomes.

🎯 What You'll Do

• Perform in-home evaluations and develop care plans
• Help patients with mobility and regain independence
• Communicate and coordinate with patients' physicians
• Evaluate and report patients' progress to MD

📋 Requirements

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
