Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes, performing evaluations, developing treatment plans, and educating patients and families. This role allows patients to receive medical care at home while you work as part of a multi-disciplinary team.

Requirements

• Current Speech Language Pathologist license in OH
• Certificate of clinical competence from ASHA or equivalent educational and work experience
• Solid computer skills; experience with electronic medical records preferred
• Prior home care experience is a plus but not required

Responsibilities

• Make home visits to clients in designated territories
• Perform evaluations to assess and treat speech, language, communication, or swallowing difficulties
• Develop treatment plans based on evaluation and physician orders
• Educate patients and families on rehabilitative care and independent living
• Document observations and interventions using electronic touch pad tablet
