You'll join a mission-driven team transforming senior mental health care by serving as a trusted partner for older adults on Medicare. You'll tackle complex care challenges and help connect the dots between mental, physical, and social health. 🎯 What You'll Do

• Build trusted relationships with older adults and their families
• Create individualized care plans addressing clinical and social needs
• Collaborate with therapists and care coordinators on patient care
• Provide feedback to shape Sailor's patient advocacy model

📋 Requirements

• Active Registered Nurse license in good standing
• Experience in healthcare advocacy or care navigation with Medicare populations
• Familiarity with SDOH and how they affect patient outcomes
