Description

As a Licensed Practical Nurse (LPN) with BAYADA Home Health Care's Jefferson Health at Home by BAYADA office, you will join a multi-disciplinary team providing skilled nursing and rehabilitative care to clients in their homes, enabling them to receive medical care while remaining at home. You will make home visits, administer medications, perform wound care, monitor clients, and document care using electronic medical records.

Requirements

• Current LPN license in state of practice
• Minimum one year recent verifiable LPN experience
• Graduation from an accredited nursing program
• Ability to travel to cases as assigned
• Solid computer skills; prior EMR experience preferred

Responsibilities

• Make home visits to clients in designated territories
• Administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes to Clinical Manager
• Execute and document doctors' orders
• Perform client assessments and coordinate case management
