Description

You will provide home health visits to adult and geriatric patients in Columbia County as a Speech Language Pathologist, working with a multi-disciplinary team to deliver rehabilitative care that helps clients remain at home.

Requirements

• Current Pennsylvania Speech Language Pathologist license
• Certificate of Clinical Competence in Speech from ASHA or equivalent education/experience
• Solid computer skills; prior EMR experience preferred
• Prior home care experience a plus, not required

Responsibilities

• Conduct home visits for speech, language, communication, or swallowing evaluations
• Develop and implement treatment plans based on evaluations and physician orders
• Educate patients and families on rehabilitative care and safety
• Document care using electronic medical records (EMR) via touch pad tablet
• Participate in multidisciplinary care planning and revisions
