Description

As a home care Registered Nurse, you will provide rehabilitative care and skilled nursing to clients in their homes, allowing them to receive medical care while staying at home. You'll follow care plans, administer medications, perform wound care, monitor conditions, and coordinate care as part of a multi-disciplinary team.

Requirements

• Current RN license in NJ
• Minimum one year recent clinical (medical/surgical) experience
• Graduation from an accredited nursing program
• Ability to travel to cases
• Strong interpersonal and computer skills

Responsibilities

• Follow designated care plans for patients
• Perform home visits to administer medication, wound care, treatments, and procedures
• Monitor clients' conditions and report changes
• Perform client assessments and case management
• Document observations and interventions using electronic medical records
