You'll manage a caseload of high-need individuals using evidence-based engagement strategies and relationship-based care coordination. You'll support whole-person health goals while collaborating with a multidisciplinary team to reduce emergency visits and hospital admissions.

🎯 What You'll Do

• Maintain ongoing caseload using evidence-based approaches
• Support members with social service navigation and health goals
• Conduct periodic telephonic and SMS outreach
• Coordinate care with multidisciplinary team and community partners

📋 Requirements

• 1+ years of general work experience (case management preferred)
• Bilingual English/Spanish fluency
• Reliable transportation and valid driver's license
• Strong technical skills with CRM, Excel, Word, and video conferencing

✨ Nice to Have

• Lived experience working with individuals experiencing homelessness or SMI/SUD
• Experience in Medicaid or Enhanced Care Management
