💼 About This Role

You'll serve as a frontline clinical navigator for members diagnosed with cancer, conducting telephonic assessments and care coordination. You'll help improve service offerings by communicating member feedback to clinical leadership. This role offers the opportunity to work remotely or hybrid from Nashville.

🎯 What You'll Do

• Conduct telephonic clinical assessments and medication reconciliation.
• Coordinate care transitions and discharge plans with hospital case managers.
• Monitor member progress and establish targeted support plans.
• Provide referrals to PT, OT, skilled nursing, palliative care, etc.

📋 Requirements

• Active unrestricted compact Registered Nurse (RN) license.
• 5+ years of nursing experience with 3 years high-acuity adult oncology.
• Oncology Certified Nurse (OCN) or equivalent certification.

✨ Nice to Have

• BSN preferred.
• Experience with video chatting, Google Suite, Slack, electronic health records.
