Description

You will play a critical role in our whole-person, interdisciplinary care model, directly outreaching and engaging with individuals living with Serious Mental Illness/Substance Use Disorder, experiencing homelessness, or having high medical needs. Leveraging AI and automation to reduce administrative burden, you'll coordinate care to close gaps and improve outcomes for high-need patients.

Requirements

• Bilingual in Spanish and English
• Experience working with underserved populations, including SMI/SUD and homelessness
• Motivated, creative, and empathetic approach to care
• Ability to work remote and travel for in-person visits
• Commitment to whole-person, community-driven healthcare

Responsibilities

• Outreach and engage with individuals with Serious Mental Illness/Substance Use Disorder, homelessness, or high medical needs
• Conduct on-site community visits 2-3 times per month
• Coordinate whole-person care (clinical, behavioral, social) with interdisciplinary team
• Build trust and relationships to engage underserved populations
• Utilize AI and automation tools to streamline care coordination
