💼 About This Role

You'll provide skilled home health physical therapy to help patients regain independence, evaluating patients and developing care plans. You'll coordinate with physicians and supervise PTAs across the Bay Area. This role offers autonomous scheduling and targeted territories to minimize driving.

🎯 What You'll Do

• Perform evaluations and develop care plans for home health patients.
• Implement physical therapy treatments to restore function and prevent disability.
• Supervise Physical Therapy Assistants per agency standards.
• Coordinate with physicians and report patient progress.

📋 Requirements

• Current CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.

• High acuity patient experience.
• Case management fundamentals.
