💼 About This Role

You'll create and execute patient care plans and lead a team of 2 nurses to prevent re-hospitalization in a home health setting. You'll conduct comprehensive assessments and provide hands-on care including wound care, IV infusions, and medication education.

🎯 What You'll Do

• Perform comprehensive patient assessments and develop care plans
• Case manage patient episodes with interdisciplinary clinical team
• Supervise and mentor 2 nurses executing care plans
• Administer IV medications and manage complex patient needs

📋 Requirements

• Minimum 1 year of nursing experience required
• Current and unrestricted CA RN license
• Current BLS/CPR certification
• Valid CA Driver's License

✨ Nice to Have

• Two years of home health case management experience preferred
• Experience supervising and mentoring nurses
• Experience with high acuity patients
