You'll serve as Waymark's frontline presence in the community, connecting patients with Medicaid benefits to social support and healthcare resources. Your core impact will be helping patients overcome barriers like homelessness and hunger to achieve their health goals. This role offers a chance to work with a multidisciplinary team in a mission-driven healthcare startup.

🎯 What You'll Do

• Conduct needs assessments and set health goals with patients
• Connect patients to community resources like food banks and housing assistance
• Manage relationships with healthcare providers and care team members
• Document patient encounters in technology systems accurately

📋 Requirements

• Organized with ability to work independently and manage schedules
• Interpersonal communication and active listening skills
• Computer literate with experience using technology for documentation
• Reside within commutable distance of Woodstock, Illinois

• Community Health Worker certification
