💼 About This Role

You'll be an integral member of a multi-disciplinary team, making home visits to clients in York and Adams Counties. You'll perform thorough evaluations and develop individualized intervention plans to promote independence and achieve favorable outcomes. This role uses a state-of-the-art touch pad tablet for documentation.

🎯 What You'll Do

• Perform thorough evaluations of patients' functional status.
• Develop individualized intervention plans to achieve favorable outcomes.
• Identify equipment needs and environmental adaptations.
• Educate patients and family members on rehabilitative care.

📋 Requirements

• A current Pennsylvania Occupational Therapist license.
• A minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited by the AMA and AOTA.
