Description

You will provide rehabilitative speech therapy to adult and geriatric patients in their homes throughout Fairfax County, VA, as part of a multidisciplinary care team.

Requirements

• Current Speech Language Pathologist license in VA
• Certificate of Clinical Competence in Speech from ASHA or equivalent
• Solid computer skills; EMR experience preferred
• Prior home care experience preferred but not required

Responsibilities

• Perform home visits for evaluations and therapy interventions
• Develop and revise treatment plans based on evaluations and physician orders
• Educate patients and families on rehabilitative care
• Document observations and interventions using a tablet
• Participate in multidisciplinary care planning
