Description

In this role, you will provide home-based physical therapy to help clients regain independence. You'll perform assessments, create treatment plans, and educate patients and families, all while being supported by a collaborative team and comprehensive mentorship program.

Requirements

• Current Hawaii PT license or eligibility
• Graduation from an accredited PT program
• Comfort with EMR systems (training provided)

Responsibilities

• Perform mobility, strength, balance, and safety assessments
• Create individualized treatment plans using evidence-based interventions
• Provide gait training, manual therapy, balance work, and therapeutic exercise
• Implement skin and wound-prevention strategies
• Educate clients and families for long-term success
