Description

You will deliver high-quality telehealth care focused on prevention and chronic disease management for older adults, provide feedback on clinical protocols, and partner with care coordinators to develop personalized care plans. This is a part-time, remote role with flexible scheduling.

Requirements

• MD/DO in Internal Medicine, Family Medicine, Geriatrics, or Lifestyle Medicine
• Active medical licenses in CA, TX, and/or PA
• 5+ years clinical experience
• 2+ years telehealth experience with high EMR proficiency
• Strong foundation in preventive and lifestyle-focused care

Responsibilities

• Deliver telehealth care focused on prevention, chronic disease management, and lifestyle interventions for older adults
• Provide feedback on clinical protocols, documentation standards, and scalable workflows
• Use EMR for documenting patient information and communicating with other providers
• Partner with care coordinators, health coaches, and clinicians to develop personalized care plans
