Description

As a member of the Care Navigation team, you will deliver coordinated, patient-centered virtual care management by telephone, helping members with chronic or serious illnesses navigate the healthcare system, improve health outcomes, and reduce costs. You will lead outreach to high-risk individuals, conduct assessments, and provide compassionate follow-up care.

Requirements

• Active RN license in a compact state with ability to be licensed in other states
• 1+ years experience in case management
• 2+ years experience in a hospital or clinical setting
• Bilingual (English/Spanish) highly preferred
• Competence with multiple computer/medical record systems and Google suite

Responsibilities

• Deliver coordinated, patient-centered virtual Care Management by telephone to improve health outcomes
• Lead outreach to high-risk individuals and complete care management assessments and care plans
• Use the Nursing Process and broad knowledge to support members with pharmacy, crisis interventions, and disease-specific education
• Help members navigate the healthcare system, providing education and empowerment
• Assist during acute healthcare episodes with coordinated Case Management
