Description

You will provide rehabilitative care and skilled nursing to adult and geriatric patients in their homes in Northeast Baltimore County. As a vital member of a multi-disciplinary team, you'll perform evaluations, develop care plans, and educate patients and families to promote independent living.

Requirements

• Current Maryland Occupational Therapist license
• Minimum one year recent work experience as an OT
• Graduation from accredited occupational therapy program
• Strong interpersonal and organizational skills
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits in designated territories
• Perform functional status evaluations including ADL and IADL performance
• Develop and revise individualized care plans
• Evaluate home environment for hazards and equipment needs
• Document patient care using touch pad tablet EMR
