Description

As a Primary Care Physician at Seen Health, you will provide holistic, participant-centered care for frail and elderly adults within the PACE program. You will work with an interdisciplinary team to manage complex medical, social, and functional needs, and collaborate with technology and operations teams to improve care workflows. This role offers the opportunity to help build a culturally-sensitive, scalable care model for underserved seniors.

Requirements

• MD or DO with active board certification in internal medicine, family medicine, or geriatrics
• Current unrestricted medical license in California
• Active state CDS certification and DEA
• Enrolled or eligible for Medicare and Medi-Cal provider services
• Conversational fluency in Chinese or Cantonese preferred

Responsibilities

• Provide comprehensive primary care to PACE participants
• Conduct health assessments and develop individualized care plans
• Manage acute and chronic conditions and coordinate specialty care
• Collaborate with interdisciplinary team in care planning and daily huddles
• Partner with tech and operations teams to innovate care workflows
