🎯 What You'll Do

• Conduct initial and follow-up assessments on flagged residents
• Train and coach staff on Circadia integration into clinical workflows
• Interpret trend-based insights to prioritize high-risk patients
• Complete documentation and charts within the same day

📋 Requirements

• Certified and licensed Nurse Practitioner in ME
• 2+ years NP experience with complex adult populations
• Eligible for Medicare participation
• Valid ME driver's license and local travel ability
