You'll collaborate with a dedicated clinic team to support a patient panel alongside Primary Care Providers, ensuring high-quality care for the geriatric population in San Gabriel Valley. You'll gain hands-on experience in a culturally-focused clinic setting. This role offers equity as part of a founding team package.

🎯 What You'll Do

• Triage inbound communications from patients and partners
• Administer clinical procedures like wound care and vaccinations
• Support post-appointment processes such as diagnostic orders
• Reinforce education on care plans and disease management

📋 Requirements

• LVN license in California
• Proficiency in Mandarin and/or Cantonese
• Graduate of accredited LVN program
• Reliable transportation for home visits
